Health Insurance
Superior offers two plan choices. Plan A is a $500/$1000 deductible for individual/family. Plan B is an HRA with a $2,000/$4,000 deductible with Superior paying the first $1,000 in medical expenses for individual or the first $2,000 in medical expenses for family. Both plans offer an 80/20 coinsurance after the deductible has been met.

Dental Insurance
Superior offers an employee, employee plus one and family coverage options.

Life Insurance
Superior provides employees with a basic life insurance policy to which additional voluntary life insurance may be added at a cost to the employee.

Short-Term & Long-Term Disability Insurance
Superior provides both basic short and long term disability insurance at no cost to the employee. Additional long-term disability insurance is available at a cost to the employee.

Accidental Death & Dismemberment Insurance
AD&D coverage is in conjunction with the employee group term life insurance.

Dependent Life Insurance
Superior provides dependents of employees a group term life insurance policy. This policy provides a $2,000 benefit in the event of the death of your spouse or a dependent child.

Voluntary Life Insurance
Employees may purchase additional term life insurance on a voluntary payroll deduction basis. Converge may be elected in increments of $10,000 up to a guaranteed issue amount of $50,000 to a maximum of five times your basic annual earnings. You may elect coverage for your spouse that is up to or equal to your amount. The spouse guaranteed issue amount is $25,000. You may elect coverage for your dependent children at $10,000 each.

401(K) Plan
Superior maintains a 401(K) plan for the benefit of eligible employees and their beneficiaries. This plan permits all eligible employees to defer payment of taxes on a portion of their wages by electing to have that portion placed in investments selected by you rather than receive it as current taxable wages. Superior will match ¾ of your contribution, to a maximum of 3%. An employee becomes eligible to participate when he/she has completed 30 days of service and has reached age 18.

Flex Plan
Superior sponsors a 125(K) Cafeteria Benefit Plan, which allow eligible employees to set aside pre-tax dollars to pay health premiums, medical expenses, and dependent care expenses. Associates are eligible the first of the month following 30 days of employment; thereafter associates must enroll prior to January 1 of each plan year.

Paid Holidays
Superior observes seven paid holidays for full time, regular associates. The associate must be employed for 60 days before becoming eligible for holiday pay.
